Primary Home Care

Primary Home Care (PHC) is a medically related non skilled service that is prescribed by a physician. PHC is available to assist eligible Medicaid patients that are functionally limited in performing activities of daily living. In addition, the program includes the following in-home services:

Personal Care
  • Bathing
  • Dressing
  • Preparing Meals
  • Exercising
  • Grooming
  • Caring for Routine Hair & Skin Needs
  • Taking Self-Administered Meds
  • Toileting
  • Transferring/Ambulating
 
Home Management
  • Changing Linens
  • Housecleaning
  • Laundering
  • Shopping
  • Washing Dishes
Who Is Eligible?
  • Private Pay
  • Eligible for Medicaid
  • Medical Need for Personal Care
  • Doctor's Order for PHC Services
  • Individuals who are willing to pay privately
